Updated PennDOT Forms 10/24
There have been some updates to DMV forms, please see the forms below. If you have pre-printed forms in the office, destroy old forms and replace with the updated forms.
DL80- Form Correction to display correct fees for Dup camera card with Class M
MV140/MV371– Updated to include the statement below for retired Status applications
***NOTE: 100% of a veterans’ benefit payment or payment to the unmarried surviving spouse, upon the death of a veteran, is entitled to receive should not be included as income for the reduced Retired Status fee.
MV70S –there is a minor change to page 5, with the addition of the replacement Certificate of Exemption documents
